Home and Community Support Program
Continuum of Colorado’s Home and Community Support (HCS) Program serves children and adults in their own homes and communities through a variety of services. This program is the only in our agency that is licensed as a Home Care Agency and is approved to provide Class B services.
- We offer in-home, in-person services (we do offer some telehealth for certain approved services).
- Our service area includes the front range, from Ft. Collins to Pueblo including the suburbs and surrounding areas of Denver/Aurora/Colorado Springs Metro.
- All our Home and Community Support Providers (HCSPs) are employees of Continuum of Colorado and complete all required state and agency Person-Centered Trainings specific to the people they serve.
What We Do
Our Home and Community Support Program is dedicated to employing the best providers, regularly utilizing the Family Caregiver Act to hire family members and family referred applicants to provide person-centered services.
Personal Care
- Support with grocery shopping, appointment assistance, general money management skills, and completing Activities of Daily Living (ADLs). This service is for adults on the SLS waiver only.
Respite
- Supervision for the person receiving services to give relief or a break to the primary caregiver(s).
Mentorship
- Support with developing self-advocacy skills.
Supported Community Connections
- Support with developing skills to make connections within the person’s community. This service is for adults on the SLS Waiver only.
Homemaker (Basic and Enhanced)
- General housekeeping tasks such as cooking, cleaning, and laundry provided for the individual in their own home. Homemaker Basic is provided for the individual whereas Homemaker Enhanced is provided to include a teaching element with the individual’s participation to attain independence in these tasks.
Community Connector
- 1:1 services in the community to facilitate connections. This service is for children on the CES waiver only.
Massage Therapy
- Home-based services performed by a Colorado licensed massage therapist. This service requires a Letter of Recommendation (LOR) from a medical professional and is available to people on the Children’s Extensive Support (CES) and Supported Living Services (SLS) waivers. Massage Therapy is the systematic manipulation of the soft tissues of the body, (including manual techniques of gliding, percussion compression, vibration, and gentle stretching) for the purpose of bringing about beneficial physiological, mechanical and/or psychological changes.
Movement Therapy
- Home-based movement services performed by a board-certified Movement Therapist. This service requires a Letter of Recommendation (LOR) from a medical professional and is available to people on the Children’s Extensive Support (CES) and Supported Living Services (SLS) waivers. Movement Therapy works with the family/individual to determine an appropriate goal(s) that will use music and/or dance as a therapeutic tool for the habilitation, rehabilitation, and maintenance of behavioral, developmental, physical, social, communication, pain management, cognition and gross motor skills.
Music Therapy
- Home-based music services performed by a board-certified Music Therapist. This service requires a Letter of Recommendation (LOR) from a medical professional and is available to people on the Children’s Extensive Support (CES) and Supported Living Services (SLS) waivers. Music is used within a therapeutic relationship to address the physical, emotional, cognitive, and social needs of people. After assessing the strengths and needs of each person, the qualified music therapist provides the indicated treatment including creating, singing, moving to, and/or listening to music. Through musical involvement in the therapeutic context, people’s abilities are strengthened and transferred to other areas of their lives. Music therapy also provides avenues for communication that can be helpful to those who find it difficult to express themselves in words. Research in music therapy supports its effectiveness in many areas such as: overall physical rehabilitation and facilitating movement, increasing people’s motivation to become engaged in their treatment, providing emotional support for people and their families, and providing an outlet for expression of feelings.
Who We Are
Angie, MM, MT-BC, Music Therapist
- Angie is a board-certified master’s level Music Therapist who has been with Continuum of Colorado since 2017. Within sessions, Angie applies neuroscience to inform therapy as well as using client preferences and choices throughout. When something is enjoyable and engaging (including challenges), we are more likely to learn and grow from it.
Brandy, Licensed Massage Therapist (LMT)
- Brandy is a licensed Massage Therapist and has been with Continuum of Colorado since September of 2022. She has experience in teaching and performing Swedish/Deep Tissue, Reflexology, Cranial Sacral Therapy, and Hydrotherapy. Brandy loves her work, stating, “I love massage! I love everything about it. I love helping people to feel better, to relieve stress and tension, to see the smile on someone’s face after so much pain and discomfort. I love the simple, quiet moments in which people can take a deep breath and just simply be present in their mind and body. Mother Theresa said, ‘Do small things with great love.’ This is exactly how I try to live out my massage practice. Massage is a series of small movements, that when done with great love and intuition, make a great impact.”
Catie, MT-BC, Music Therapist
- Catie is a board-certified Music Therapist who has been with Continuum of Colorado since August of 2023. Catie got into music therapy because of her love for music, psychology, and helping people. Catie is BLS certified and has participated in Welle training. She enjoys finding new music to expand her musical horizons.
Cham’pel, Licensed Massage Therapist (LMT)
- Cham’pel is a licensed Massage Therapist and has been with Continuum of Colorado since February of 2023. She holds certifications in neuromuscular massage, trigger point therapy, and integrative techniques. Cham’pel focuses on the healing, well-being, and pain relief aspects massage offers.
Jaclyn, Licensed Massage Therapist (LMT)
- Jaclyn has been with Continuum of Colorado as a licensed Massage Therapist since April of 2023. She found her passion for massage through her own experience with its healing properties after a surgery (where other methods failed to help her recover). Her passion for helping others inspired her to complete her hours for licensure and a certification in neuromuscular massage therapy.
Lauren, Licensed Massage Therapist (LMT)
- Lauren has been a licensed Massage Therapist with Continuum of Colorado since February of 2023. She is certified in chair massage, prenatal massage, myofascial techniques, deep tissue, sports massage, Swedish massage, and is currently completing certification in Trauma Touch Therapy and oncology. A takeaway from the pandemic for Lauren was, “I realized that chiropractic care and massage therapy were really important self-care for me and that I was really passionate about helping people feel much better in their bodies. So, I took steps to apply for Massage Therapy school in order to provide the kind of care that I have received in the past and present to maintain my own health.”
Rebecca, MS, R-DMT, Movement Therapist
- Rebecca is a registered Dance/Movement Therapist (DMT) who earned her Master of Science in Arts Therapy from Pratt University. She has been a Movement Therapist with Continuum of Colorado since August of 2020. DMT is different from both OT (occupational therapy) and PT (physical therapy) in that it is very intentionally oriented toward creativity, emotions, improvisation, and connection. It is not that OT and PT do not provide these things; however, DMT is very specifically focused on these elements as they can be expressed through the body. Rebecca comes to the client with a very open mind, observing and sensing what they need, not as much with a formal curriculum or specific skills that must be attained. She sees the body as a means of communication and connection first, and as a vehicle to do things, second. DMT is similar to Music Therapy, Art Therapy and Drama Therapy, and Rebecca often uses instruments, music, singing, imaginary play and art in her sessions. However, as a DMT, she interprets how to help clients through the lens of their body: body language, eye contact, facial expression, small movements of hands, the way they shape their bodies when they sit/stand/move, whether their body is tight or loose, open or closed, whether they put weight on their whole feet (if they can stand), what kinds of habitual movements do they do, etc.
Seth, Licensed Massage Therapist (LMT)
- Seth graduated from the Colorado Institute of Massage Therapy (CIMT) in the fall of 2022. He is a Licensed Massage Therapist specializing in Neuromuscular Therapy to help his clients find relief and relaxation. Since joining Continuum of Colorado in November of 2022, he has been able to combine his passion for helping others with his expertise in massage therapy. From a young age, he felt a strong desire to make a difference, especially in the lives of those with intellectual and developmental disabilities, and he is grateful to be able to do so in his current role.
Sadie, MT-BC, Music Therapist
- Sadie received her Bachelor of Music in Music Therapy from Wartburg College in 2020 and completed her internship and board certification in 2021. Sadie moved to Colorado and joined the Continuum of Colorado team in 2023. She specializes in Neurologic Music Therapy. Much like other therapies, there are different types of music therapy and theoretical orientations that music therapists draw from. Neurologic Music Therapy (NMT) focuses on understanding how music is processed in the brain and how it can influence or change non-musical brain function and behavior. Using a research-guided system, standardized techniques are applied to meet client needs. These techniques are adaptable, and treatment is individualized based on each person’s musical preferences and goals. Sadie states, “I chose a career in music therapy because I love music, and I am passionate about helping people in a holistic way.” When Sadie isn’t providing Music Therapy she is working on earning her master’s degree in clinical mental health counseling through Adams State University.
David, Licensed Massage Therapist (LMT)
- David is a certified massage therapist and neuromuscular therapist, having earned his certification at the Colorado Institute of Massage Therapy (CIMT) in Colorado Springs. He also holds degrees in pre-med and biology from the University of Montana. His passion for massage therapy stems from his desire to help others improve their well-being and quality of life. Over the past five years, David has been proud to work with Continuum of Colorado where he focuses on supporting individuals in need, helping them reach their goals through personalized care.
Jennifer, Licensed Massage Therapist (LMT)
- Jennifer is a licensed massage therapist who graduated from the Colorado Institute of Massage Therapy in Fall 2021. She holds certifications in Swedish, Sports, Integrative, and Neuromuscular/Trigger Point modalities and has received additional certification in spa massage from Hand and Stone. Jennifer’s passion for the medical side of massage therapy developed during her time at Hand and Stone, where she helped clients alleviate pain from surgeries, injuries, and other impairments. Since April 2024, she has been a part of the Continuum of Colorado team, focusing on treating bodily misalignments and improving overall well-being. Jennifer is dedicated to making a positive impact on the lives of those she interacts with.
