Home and Community Support Program
Continuum of Colorado’s Home and Community Support Program is a licensed Home Care Agency established in 2012 providing quality services to adults and children by trustworthy and caring providers.
Continuum of Colorado’s Home and Community Support Program is dedicated to collaborating with the individual and families in employing the best providers for individuals, regularly utilizing the Family Caregiver Act to hire family members and family referred applicants to provide a person-centered care plan which allows the individual to be an active participant.
- All employees complete required trainings specific to the individuals’ needs.
- We offer in-person services, as well as telehealth/video for certain approved services.
- We currently serve the Denver/Aurora Metro area and surrounding suburbs, the Colorado Springs Metro area and surrounding suburbs, as well as the Fort Collins and surrounding suburbs.
We offer a range of services
Personal Care
Support with grocery shopping, appointment assistance, general money management skills, and completing Activities of Daily Living (ADLs) (For adults on the SLS waiver only)
Respite
Supervision for the individual to give relief to the primary caregiver(s)
Mentorship
Support with developing self-advocacy skills
Supported Community Connections
Support with developing skills to make connections within the individual’s community
Homemaker (Basic and Enhanced)
General housekeeping tasks in the individual’s primary residence, including cooking, cleaning, and laundry
Community
Connector
One-on-one services in the community to facilitate connections (Children Extensive Support waiver only)
Massage Therapy
Physician referred home-based services performed by a licensed massage therapist
Movement/
Music Therapy
Physician referred home-based services performed by a board certified Music or Movement Therapist
