Our Residential Department delivers a Person-Centered approach to providing services. We offer a variety of living options with supervision and independence levels tailored to each person’s strengths and support needs. As a part of a holistic approach, the Residential Department encourages involvement with family and friends. We are committed to addressing the total needs of each person in a typical home environment.

What We Do

We provide residential options to adults on the DD Waiver in a variety of settings.

Host Homes

  • Our host homes offer quality supervision, supports, and services tailored to each person’s needs. Our host home providers are independent contractors that are certified in the same required trainings as our Direct Support Professional staff. Each person is integrated into the host home only after a robust matching process has identified a true match. Each provider and person looking to live in a host home are matched based on choice, preferences, and level of support needed. The host home provider then provides Person-Centered services to the people living in the home.

Family Caregiver

  • Similar to our host home settings, our family caregivers are independent contractors that deliver the Person-Centered Plan to their own adult children (or family member) with I/DD right in their own homes. This service is an option for anyone on the DD waiver and has a family member that would like to provide the services for their loved one. PCA Homes

  • Our Personal Care Alternative (PCA) Homes offer a living option for adults on the DD waiver that have significant medical, behavioral, or activity of daily living needs. People living in a PCA are encouraged, with the help of 24-hour staff, to be as independent as possible through each person’s individualized Person-Centered Plan. Staff assist each person with all activities of daily living, including proper nutrition and food preparation, shopping, personal and hygiene care, transportation, medical appointments, and community events with the goal of increasing each person’s independence.

Apartments

  • We offer staffing support to people on the DD waiver living in their own apartment (either by themselves, or with a roommate). The goal of our support in this program is to maintain and continue to help people live as independently as possible. Some of the tasks staff might assist with range from meal management, grocery shopping, medical appointments, medication management, or simply providing transportation to different places in the community.

Who We Are

All our Host Home Providers, Family Caregivers, and Direct Support Professional staff are provided with comprehensive and Person-Centered training prior to working with any of the people we serve.
